To become a member of the AOSSM, orthopaedic surgeons, physicians and allied health
professionals must devote a significant percentage of his or her practice to sports
medicine research and educational activities. Such activities may include service as a team
physician at any level of competition, educating persons involved with the health of athletes,
service to local, regional, national and international competitions, and the presentation of
scientific research papers at sports medicine meetings.

| Applying for Active Membership |
Deadline: November 1, 2010 |
Active members are: U.S. or Canadian citizens who are Orthopaedic surgeons specializing in sports medicine. |
| Applying for Associate Membership |
Deadline: November 1, 2010 |
| Associate members are either 1) international orthopaedic surgeons practicing outside of the
United States or Canada who are certified in orthopaedic surgery and members of their country's
national orthopaedic organization or 2) osteopaths not qualified to be Active Members but
certified by the American Osteopathic Board of Orthopaedic Surgery. |
| Applying for Affiliate Membership |
Deadline: November 1, 2010 |
| Affiliate members are physicians, surgeons, or allied professionals not qualifying for other
membership categories who are interested in or contributing to the knowledge of sports medicine.

| Applying for Upgrade to Active or Associate Membership |
Deadline: November 15, 2010 |
| Upgrade applicants are current AOSSM Candidate members who meet the requirements of Active or Associate membership. |
| Applying for Candidate Membership |
Deadline: December 15, 2010 |
Candidate members are physicians who are enrolled in or have completed an ACGME-approved
orthopaedic sports medicine fellowship in the United States or Canada.
