Applying for Affiliate Membership
Please review the Society's membership classifications and be certain that you qualify before
proceeding. Affiliate members are physicians, surgeons or allied professionals not qualifying for
other membership categories who are interested in, or contributing to, the knowledge of sports
medicine.
Prior to applying for membership, you must fulfill the following requirements:
- Be practicing in your current geographic location for two or more years;
- Must have attended at least one AOSSM Annual Meeting in the past five (5) years.
To be considered, applicants need to provide demographic, practice and educational information
and document their involvement in sports medicine through:
- Service as team physician at any level of competition;
- Service to local, regional, national and international athletic competitions;
- Education of persons providing health care to athletes;
- Continuing medical education in sports medicine;
- Research and publication; or
- Submission and presentation of papers at sports medicine functions.

Membership Application Materials
If you meet the criteria and would like an application, you will need to proceed with the following:
- Complete the Affiliate Membership Application and forward it to the Society office. You may
submit the application electronically or by mail.
If you would like to submit your application electronically please follow the instructions:
save the Affiliate Application to your computer, complete and email the application to the
AOSSM Membership Department.
The $150.00 application fee can be paid by check (payable to the AOSSM) or via credit card.
- Affiliate member applicants must be sponsored by five Active Members (Active Member roster) of
the Society, preferably from within your state or practice area. No more than one may be an
associate of yours.
Please remember, the Active Member list does not include all members of the Society, as only
Active Members may sponsor applicants.
- Forward the Sponsorship Form to the five Active members (Active Member roster)
of the Society who will serve as your sponsor. Sponsors are asked to submit the Sponsor form and Sponsorship letter.
- Forward the Team Service Request Form to each team you are currently providing service for
verification of your involvement.
The applicant is responsible for contacting all references and sponsors. Your application is not
complete until the Society office has received your sponsor letters, completed team forms/letters,
completed application and application fee.
The application deadline is November 1st. ALL application materials, including references must
be received in the Society office by that date.
